The state of Illinois’ normal fee is $15.00. You must also pay an additional $8.50 handling fee plus a $19.50 fee for UPS service inside the continental United States. If you are ordering from an international location, additional fees still apply. You will still have to provide supporting documents that prove your identity to get your replacement birth certificate.
Faxed Ordering
You can request an official copy of your birth certificate as well. Although available, this is not the most ideal way to order your birth certificate. The sensitivity of the documents that must be submitted makes it much less desirable to order your documents via fax. The fee is $15.00 plus a $12.00 UPS fee. UPS will not deliver to post office boxes so you will need to list a physical address.
