In Person Birth Certificate Request
Service for walk in clients is available from Monday to Friday from 7:45 AM up to 2:30 PM. When applying in person, one must fill out all the important details on the request and application forms. In getting a certified true copy of one’s birth certificate, the charge will be $10.00 for the first copy. But if multiple copies are ordered at the same time, the charge per additional copy will only be $4.00. If you applied as walk-in, payment should then be in the form of a cash, money order, certified check or manager’s check. Fees are non-refundable but they may be paid in advance. In special cases where there are no records to be found, the fees are put on hold to cover the cost of the search. However, if there are any changes you would like to do once order has been processed and received, an additional charge of $10 must be paid to accommodate your request.
A certified true copy of your requested birth certificates will not be released on the same the application is made. They may be available for pick up after 10 working days once the request has been approved. In special cases, a replacement birth certificate can be released the same day, in these same day requests you must present a written documentation establishing the need of urgency and must be approved by the officer in charge.

For mail requests, one must send the application forms together with a photocopy of applicant’s government issued photo ID. The Department of Health will not accept any cash payments and personal check fees for mail requests. All mail requests with personal checks and cash will be rejected and returned to sender. Payment will be the same as mentioned for walk in applicants. You may pay in the form of certified check or manager’s check or money order. They should be made payable to State Department of Health.
A certified true copy of the applicant will be sent out by mail around 2 to 3 weeks after the approval and receipt of such application.
Order Hawaii Birth Certificate Online
For an online request, application forms can be requested by logging on to www.ehawaiigov.org/ohsm. Payments for online requests must be charged to a valid credit card. Aside from the $10.00 charge per replacement copy request, an additional charge of $1.50 per copy applies to all online requests. Also, a $0.25 charge applies in addition to the $4.00 fee per additional copy. The certified true copy of one’s birth certificate will be sent out by mail after 2 to 3 weeks from the time you made a request.
