Like many states, Washington does not accept online orders but uses a third party company called VitalChek Network as many other states do. VitalChek is the official government authorized service. Mail-In to Request Washington Birth Record

Ordering a certified copy of your birth certificates by mail is easy. Just complete the required form and submit your request to; Center for Health Statistics-Department of Health, P.O. Box 9709 in Olympia, WA 98507. Make checks and money orders payable to “Department of Health”. If you need expedited delivery you can chose express mail of set Federal Express. Express mail is an additional $18.30 if mailed to a U.S. address, this is in addition to the $20.00 fee for each certificate. Federal Express is an additional $15.00 if mailed to a U.S. address. Alaska, Hawaii, Canada or Mexico addresses cost an additional $25.00.

Phone Order a Washington Replacement Birth Record

When ordering by phone you must provide the exact information;type of credit cards you are using, credit card number, credit card expiration date, address the certificate is to be sent to. If you are requesting UPS Next Day Air service you must provide a street address, no P.O. Boxes! To order by phone please call (360) 236-4313. Certificates ordered by phone cost $31.50 each. Express mail is an additional $18.30 and UPS Next Day Air is an additional $18.50.
