In Wisconsin the Department of Health Services handles vital records. Applications for birth certificates replacements in Wisconsin must be submitted to the health department by mail, in person or telephone/fax. Unlike many other states Wisconsin does not accept applications online so your options are a little limited.
Replacement Birth Certificate Wisconsin – Apply In-Person
Applying in person is easy. Visit the office at 1 West Wilson Street, Room 158 in Madison, Wisconsin. The office is open from 8:00am to 4:15pm Monday through Friday except for state holidays and designated furlough days. Applications can be found on the website or in the office. You must bring valid identification; a Wisconsin driver’s license with photo, Wisconsin I.D. with photo or and out-of-state drivers license with photo. You can also present two of the following; a U.S. passport, checkbook/bankbook, major credit card, health insurance card, recent signed lease, government issued identification, utility bill or even a traffic ticket. All forms of identification must be valid and have current name and address.
Regular and expedited services are available. Regular services are requested within two hours of acceptance at the counter if received by 2:00pm. Request received after 2:00pm will be completed by 10:00am on the next business day. There is a high volume of request for uncertified copies. Therefore, a request for an uncertified copy could take up to three months. The cost of a search which includes one birth certificate is $20.00 additional copies are $3.00. Keep in mind that fees are non-refundable.
The Expedited service must also be done in person. Expedited services are usually completed within 15-30 minutes. Uncertified copies of birth certificates will be completed within two hours but not if received after 4:00pm. The cost for expedited services are also $20.00 but do not include a certificate search. So the total cost is $40.00 for the first copy plus the certificate search fee. Additional copies are $3.00. Again, fees are non-refundable if a certificates are not found. Cash, checks money orders are acceptable payments for in-person requests. Credit cards are not accepted

Apply for Replacement Birth Certificate By Fax
You can submit your birth certificate request by fax and the birth certificate will be sent to you through regular mail or overnight USPS. All faxed request will be charged for expedited service which is $20.00. All faxed request will be completed and mailed within 5 business days.
